 At Heartfelt Transitions, we know that every family’s journey is unique—and often, it’s not easy. That’s why we’re dedicated to providing compassionate, personalized support for seniors and their families across Arizona’s East Valley, Casa Grande, and surrounding communities.

Jana Martin grew up in a small Iowa town, where supporting her community and helping others was simply part of life. Her love for the outdoors brought her to Colorado, where she attended college, raised her family, and stayed active in community service.

Eric Dean was raised on a Colorado ranch, where he learned firsthand the value of community support and lending a hand to neighbors. His career in the communications industry took him across the country, building connections and contributing to communities nationwide.

After relocating to Arizona to be closer to family (and enjoy warmer winters!), both Jana and Eric found themselves navigating the complexities of senior care firsthand as they supported their own parents. From doctor’s appointments to researching senior living options, they experienced the stress, confusion, and emotional weight that so many families face.

Motivated by their own journeys, Jana and Eric became certified Senior Transition Specialists through Mom’s House—a respected nationwide resource for families navigating senior transitions. Their training included specialized coursework in the transition journey, sensitivity and empathy, building a trusted network of resources, and a deep dive into the senior living industry. Their ongoing association with Mom’s House means continued mentorship, education, and access to the latest resources for the families they serve.
