Jana was born and raised in a small town in Iowa, where she developed an interest in supporting her community and helping others. Her fondness for the outdoors led her to Colorado, where she attended college, raised her family, and participated in community activities.

Eric was born and raised on a ranch in Colorado, where he learned the importance of community support and assisting neighbors. His career in the communication industry took him across the country, where he contributed to telecommunication growth in various communities.

In recent years, they relocated to Arizona to be closer to family and avoid the winters. Their experience with senior care placement stemmed from their personal experiences supporting their parents. Navigating through different aspects of care, from doctors' appointments to senior living options, and understanding the complexities involved in researching and making decisions for the best care for loved ones motivated them to learn more and contribute back.

They became certified Senior Transition Specialists through Mom’s House, a nationwide resource that helps families navigate this process. Through certification, they completed specialized training in the transition journey, sensitivity training, building a network of resources, and understanding the senior living industry. Their association with Mom’s House provides support, mentorship, continued education, and resources.

Heartfelt Transitions was created to serve the seniors in the Southeast Valley, including Queen Creek, San Tan Valley, Chandler, Gilbert, and the surrounding communities. Our mission is to serve seniors by providing guidance on funding options, senior communities suited for their needs, and referrals to other senior professionals as needed. They aim to support the growing senior population of their community.
